Roux-en Y Procedure for Treatment of Complications after Subtotal Gastrectomy

Abstract
|
|
|
Complications as the unsatisfactory results after Billroth 11 type subtotal gastrectomies are due to
early pathologic changes and as likely to be due to nutritional difficulties as to physiologic changes
lately.
And various surgical procedures to improve these complications have been attempted.
The authors in the 17th. Army Hospital, experienced 4 cases of complications after the fastric
resection with retrocolic Billroth 11 reconstruction as the original operations. ie:
1st. case was 23 year old male soldier having the fecal fistula on the abdominal wall due to repe
ated reoperation,
2nd. case was 23 year old male soldier having the efferent loop obstruction with stoma stenosis,
3rd. case was 57 year old female civilian having the stoma stenosis due to chronic irritative fib
rotic change, and the last case was 37 year old female civilian having the efferent loop obstruction
due to barium fecalith.
In all instances after resected the stoma, antecolic Finney type reconstructions and the implanta
tion of the afferent jejunom into the efferent jejunum at 25㎝. below the gastroenterostomy as well
as Roux-en Y procedure were made, and obtained good results.
